This article provides guidance on how to complete the Register to disclose form. The Register to disclose form is a way for new users disclosing through CDP for the first time to request access to the CDP Portal to start their disclosure.

- The Register to disclose form will be available when we launch the disclosure cycle, week commencing June 16, 2025.
- Completing the form does not grant you immediate access to the Portal. Instead, a submitted form sends a request to CDP for its processing. Once processed, the user will be contacted by CDP with further instructions on next steps.

- The Register to disclose form is available in English, Portuguese, Japanese, Simplified Chinese and Spanish. You can view and change the language of the form on the top right of the form, beside the search option.
- We request you complete the form in English where possible to help CDP accurately match you to your organization.
- Ensure all information provided is correct when completing the form to allow CDP to process your request by matching you to the correct organization account.
- We request your patience once your form is complete; CDP aims to respond to users with an update on their submitted Register to disclose form within 5 working days, subject to the volume of queries received.
- We encourage you to complete and submit the form as early as possible, to give you sufficient time to complete your disclosure within our 2025 timeline and reporting window.
Who should complete the Register to disclose form
Any new user belonging to an organization that would like to disclose through CDP, without existing access to the CDP Portal should complete the Register to disclose form to request access to the CDP Portal.
If you have colleagues with existing access to the CDP Portal, we encourage you to contact them directly instead, to assist with adding you to your organization’s CDP Portal account. This will allow you to access the Portal faster and with fewer steps.

Information required to fill in the form
You will need the following information to complete the form:
- Work email address
- Full organization address
- For companies and public authorities: the DUNS number tagged to your organization*
- If applicable: The full organization name of the organizations/initiatives from which you are expecting a CDP request for the year (your Requesters)
- For cities – the latitude & longitude of your city
*A DUNS number, or Data Universal Numbering System number, is a nine-digit number assigned to businesses by Dun & Bradstreet, a global data provider that CDP uses as a unique identifier for all companies disclosing through CDP. Please note that cities, states, regions, and some other organizations are not assigned DUNS numbers.
Step 1: Selecting your purpose
On the form landing page, first select the purpose that applies to your organization.
- My customer(s) and/or CDP Capital Market signatories have requested my organization to disclose through CDP.
Select this option if you have received a request to disclose from your customer(s), CDP Capital Market, your bank(s), or an initiative you are part of to disclose.
When you select this option, you will need to know the full name of your Requester(s), requesting you to disclose through CDP.
On the next page, you will be asked “Who has requested you to disclose?”.
Here, please select all the Requesters from the drop-down list that you are expecting a request from for this disclosure cycle. You can filter the drop-down list by entering the full organization name of your Requester. You may select more than one option. We encourage you to provide all the Requester names you are expecting a request from to ensure CDP can accurately verify your requests and details to match you to the correct organization account.
For example, if you are expecting a request from CDP Capital Markets, filter the drop-down list by typing “CDP” and then selecting ‘CDP Capital Markets’. Note, if you tried typing in “Capital” first, you will not get the intended results as the Requester name order has to match for the options to filter correctly.
- My organization has not been formally requested to disclose by anyone but would like to disclose through CDP independently.
All organizations are welcome to disclose through CDP. Select this option if you would like to voluntarily and independently disclose through CDP and are not expecting a request from any of your customer(s).
This option also applies if you do not have a CDP Capital Markets request but would like your response to the questionnaire to be shared with all CDP Capital Markets signatories independently, irrespective of any customer request(s) received. Please visit our article on understand your requests for further information. If you are part of public authority that does not have a CDP Capital Markets request, please select this option.
When you select this option, you are requesting to be set up as a “Self-selected company (SSC)”. Disclosing as a Self-selected company means:
- You will be subject to the CDP admin fee which is payable before you submit the questionnaire. Please see our article on how to pay your CDP admin fee for further guidance
- Your questionnaire response will be shared with all CDP Capital Markets signatories. Please see our article on understanding response and score privacy for further information.
- You will be eligible to receive a score, providing you respond to the full version of the questionnaire and submit your response by the scoring deadline.
Please see our article on understanding score privacy for more information. - If you choose to submit a public response, your response may be included in CDP’s data products to Supply Chain Members.
For this reason, once you select this option, you will be prompted to note the above to confirm your intention to disclose as a self-selected company (SSC).
- I am part of a City, State or Region that would like to disclose through CDP.
If you represent a city, state or regional government that would like to disclose through CDP and require access to the CDP Portal, please select this third option as your purpose for registering.
Step 2: Provide your personal details
All users completing the Register to disclose form, regardless of purpose selected, are required to provide their first name, last name and email address. These are mandatory, free text fields.
Please ensure you provide an official work email address with a domain that matches your organization to allow CDP to process your request smoothly.
Step 3: Provide your organization details
If you are part of a city, state or region, manually enter your full organization address in the respective fields as prompted.
For companies and public authorities, you will need to use the mandatory ‘Organization Search’.
Using the ‘Organization Search’ field *(not relevant for city, state or region).
The organization search field is linked to the Dun & Bradstreet (D&B) database to help verify your organization. CDP uses DUNS numbers as a unique identifier for organizations disclosing through CDP.
To use the ‘Organization search’ to find your organization, type your organization’s name or its 9-digit DUNS number. Select your organization accordingly.
Tip: For most accurate search results, type your full organization name as registered with D&B or enter your 9-digit DUNS number directly into the search field.
Confirm your organization details
Once you make a selection from the search results, you will be prompted to confirm if you have selected the correct organization. Check the organization name and address as this is critical information used to match you to the correct account.
If the information is correct, select ‘Yes’. Your organization name and address will be automatically populated in the respective fields after.
If details of the organization are incorrect, select ‘No’ to return to the organization search and try again.
Can’t find your organization?
If your organization does not have a DUNS number or you are unable to find your organization using the organization search field, when you select ‘No’ to confirm your search selection, you will have an option to contact CDP for additional support.
You will need to raise a separate case from the CDP Help Center. To do so, you will need to sign-in or register using the link provided to the Help Center, navigate to the My Support section to raise a case.
Note: When you create a Help Center account, this allows you access to contact CDP for support. This does not grant you access to the CDP Portal which is a different system.

Step 4: Complete remaining mandatory fields and submit
Select the ‘Country of business address’ from a drop-down list of countries and provide your organization’s official business website address.
Once done, proceed to confirm the accuracy of all the information you have provided in the Register to disclose form and acknowledge CDP Privacy Notice in accordance with which CDP will process your personal data, by selecting the two check boxes.
If all mandatory fields are completed, you may proceed to submit your form by clicking on the blue ‘Confirm Submission’ button at the bottom of the page.
Once the form is submitted, you will receive a notification via email to confirm we have received your request with a case reference. You can monitor and follow up on your case or add any additional information via the Help Center, after signing in.
If you are a part of a City, State or Region and selected option 3 as your purpose, you will need to further select an option between City, State or Region from the drop-down.
A “City” includes all local governments (counties, towns, local authorities, metropolitan areas, etc.).
A "State or Region" includes all subnational governments that operate at a level below the national or federal government (states, provinces, regions, etc.) other than cities.
Once done, type the full name of your city, state or region in English in the next field.
For cities, you will need to provide the valid floating-point value for the latitude and longitude of your city.
Note: The latitude and longitude values must be between -180.00 and 180.00.
Public authorities are public sector organizations or Crown corporations created by or affiliated with a government that have a specific mandate related to providing a core public service such as utilities, transportation, housing, infrastructure, etc.
In 2025, public authorities that have not received a request to disclose by CDP Capital Markets will need to register to disclose as a Self-selected company and ask CDP to add your organization to the Public Authorities list.
For Public Authorities, CDP offers two additional questions: one to describe your organization’s legislative mandate, providing necessary context for your questionnaire response, and the other to enable you to describe projects for which your organization is seeking financing.
- If you are part of a Public Authority, please complete the Register to disclose form, selecting option 2 for your purpose i.e. “My organization has not been formally requested to disclose by anyone but would like to disclose through CDP independently.”
- Once your form has been processed, please contact CDP to inform us that you are part of a public authority and require the public authority-specific questions to be added by raising a case via our Help Center, My Support section after signing in. When you raise a case, please select ‘I have a query related to my account or organization’ as the case category and ‘I have a query related to my organization's requests’ as the sub-category.
What happens after CDP receives the submitted form?
We aim to respond to you within 5 working days after the form submission, with the next steps towards your disclosing journey. However, please note this is dependent on the volume of requests CDP receives. We request your patience as we aim to address all your queries.
Once we receive the form, CDP will manually verify all the information provided with our database. If the information provided matches our records, we will process your request by creating an account for you and/or grant you access to your organization’s account on the CDP Portal.
Once access has been granted, you will receive an automated email with a link to ‘Go to Portal’.
Note: this is a one-time unique personalized link connected to your email address provided. Please do not share it with others.
Click on the link, check your organization name and email address mentioned on the top of the page to ensure the details are correct. Then, proceed to create your account on the CDP Portal. After which, you will be able to sign in to the CDP Portal to start your disclosure journey.

If the information provided does not match our records, we may ask you to provide further information. This may mean it will take a longer time for you to get access to the Portal.
If the information provided matches our records, and there are existing contacts from your organization with access the CDP Portal, your request will be directed to them for approval. We encourage you to contact your colleagues with existing access to the CDP Portal instead of filling in the Register to disclose form, for a quicker access to the CDP Portal.
Troubleshooting issues
‘Confirm submission’ button is blocked
If you are unable to submit the form as the “confirm submission” button is blocked, it is likely due to missing information in a mandatory field marked by “*” or an error in the information entered.
Check to ensure that all mandatory fields have been filled in, with valid numbers, text, domains (for emails or websites) or selections from options where relevant and that there is no error messages.
If that does not work, try refreshing the form to complete it or contact us for further support via our Help Center.
Not receiving automated email
We advise you to check your Junk or Spam folders if you do not receive the automated emails in your inbox. Ensure the following email addresses has been whitelisted by your IT department:
Note: These email addresses are only used for automated external communications from CDP. Please do not reply or send emails to these accounts.

Can’t find your organization
As mentioned above, if your organization does not have a DUNS number or you are unable to find your organization using the organization search field, when you select “No” to confirm your search selection, you will have an option to contact CDP for additional support.
Contacting CDP
You can contact CDP via our Help Center by raising a case for our Support team from the My Support section.
To navigate to the Help Center, use the link ‘Click here for support’, you will be directed to a sign-in page for our Help Center.
Alternatively, navigate to the Homepage from the top bar and scroll the link to sign in or register.
Register and proceed to complete the necessary, verification of your email and setting a password. Once done, you can return to the same link/page to sign-in to the Help Center.
Note: this registration is for the CDP Help Center account. This does not grant you access to the CDP Portal or the questionnaire.
After signing in, select ‘My Support’ from the top options bar, and click on ‘Contact Support’.
You can proceed to raise a case to contact the CDP support team. When you raise a case, select the category: ‘I need to be added to my organization's account’ and the subcategory: ‘I would like to be added to my organization’. Please provide as much detail as possible for the CDP support team to be able to help you efficiently, including: organization name, website, DUNS, name of requesting organization (if applicable) etc.
